EMPLOYMENT AGREEMENT

_____________________________, referred to as EMPLOYER, and _____________________________, referred to as INTERN, agree:

INTERN shall intern with the _____________________ department of EMPLOYER beginning on this_____________ day of____________, 20____, and ending on the _______________ day of____________, 20____. During this period, EMPLOYER shall use its best efforts to permit INTERN to participate in the activities of the_____________________ department in a good faith effort to further the education of the INTERN.

INTERN herewith acknowledges that the copyright and all other intellectual property rights of materials created by the INTERN are created as the result of an educational relationship and all rights shall be the exclusive property of the EMPLOYER. Upon request of EMPLOYER, INTERN shall execute any documents requested by EMPLOYER in relation to such rights. The duty to execute such documents shall survive the expiration of the agreement. INTERN shall fully and completely cooperate and comply with the standards of EMPLOYER. Any failure to do so shall constitute cause for immediate termination of this agreement.

EMPLOYER may report to the educational institution which the INTERN is enrolled the evaluation of the EMPLOYER of the INTERN's performance. INTERN agrees that the EMPLOYER is entitled to do so under a privilege and herewith waives any claims related to the evaluation submitted by the EMPLOYER to the educational institution. EMPLOYER shall prepare any necessary forms of evaluation or attendance for purpose of securing academic credit for the INTERN. INTERN herewith releases and holds EMPLOYER harmless from any and all claims which may accrue during the internship for personal injury, except for intentionally inflicted injuries, during the period of the internship. INTERN should purchase health or accident insurance.

No wages or other compensation, other than the educational opportunities provided by the internship shall be paid to the INTERN. This is the entire agreement between the parties, and this agreement may only be modified in writing.
